The Community Choice Waiver provides home and community based services to eligible seniors and adults with disabilities who need assistance with personal care and daily living activities requiring nursing home level of care.
MIHC provides 24-hour live-in care with a principal caregiver chosen by the client. Caregivers assist with bathing, dressing, eating, ambulating, and toileting. Family members (excluding spouses or legal guardians) are eligible to serve as caregivers.
All caregivers receive a monthly tax-free stipend along with ongoing support and training from a registered nurse and a dedicated care manager. Annual training and criminal background checks ensure the highest standard of care.

All caregivers are required to complete a daily note for each day of active service. Your note documents care activities, client condition, and any notable observations. Please submit before midnight on each day of service.
